If you want an in person or telehealth consultation, please kindly make an appointment! If you would like an email reply, we request that you simply fill out the form below, attach photos, such as the samples shown here, and submit! It’s a fast, private way to learn about your options.

An in-office consultation will be required prior to beginning any treatment.

You will need to accept our portal invitation before the telehealth appointment. The video link will be available on the portal for check-in on onpatient.com
Dr. Roca will review your check-in forms the day before your visit. For your consultation, please have a clear glass of water, mouth tape, and Q-tips ready to use during our consult. Ideally, please sit in a chair and have a couch or bed nearby.
If you have a panoramic x-ray, sleep report, radiology report, blood work, or myo-functional therapy report, please email those prior to your appointment to family@arlingtonsmilecenter.com
